I wanted to send out a reminder about our 6th and 8th grade students having the opportunity to take the Kentucky Incentives for Prevention (KIP ) Survey on Wednesday, November 6, 2024. This survey informs school leaders about student issues, highlights concerns, and initiates action. HMS is participating in this important project in an effort to recognize and meet the health and wellbeing needs of our students.
Participation in the survey is optional, and students will not be penalized for non-participation. The survey is completely anonymous and confidential. Student names are not included and information gathered from the survey cannot be traced back to individual students. The students will complete the survey at school during a designated class period.
In order for your student to take this important survey, they will need parent permission. If they do not have a permission form completed by Wednesday, October 23, 2024, they will not be allowed to participate in the survey.
There are three ways that you can give permission for your student to participate.
1) You can click on the following link and complete the Google form. If you do not wish for your student to participate in the survey, simply choose "No" on the form.
2) You can open the following attachment, find the permission form and at the bottom of the form there is a QR code that you can scan with the camera on your phone. This will also take you to the Google form so that you can complete a digital permission form.
3) The third option would be to print out the permission form from the attached letter, sign it, and send it to school with your student. Students can turn in permission forms to Mr. Tilford in the office.
The attached letter has more details about the survey and if you would like to see a copy of the survey, there will be one available in the front office at HMS for parents to review. The permission forms will need to be completed by Wednesday, October 23.
